How do I make an appointment?

Appointments should be made directly with the therapist you choose. Call the answering service number, which is (352) 374-8526, and request a call back from the therapist you wish to make an appointment with. You should receive a call back within 24 hours or same day by the therapist, and you can schedule an appointment directly. If you have questions before making an appointment, you can ask them at the same time.

Can I schedule an appointment in the evening hours or on Saturdays?

We do offer evening appointments, and Mr. Frazier has some Saturday appointments available.

What do I need to bring to my first appointment?

Bring your insurance information including your insurance card. You will be asked to fill out some paperwork and the therapist will look at your insurance card and take down the necessary information or billing purposes. If you are not using insurance, you will still have some paperwork to fill out prior to starting your session.

Who do I check in with when I get to our office?

Just have a seat in the waiting room and your therapist will come out to get you when they are ready to begin your session.

How do I know how much my copay will be and when do I pay it?

We highly recommend that you call your insurance carrier prior to your first appointment and find out what your policy’s mental health benefits are. Specifically, you should find out how many sessions per year your policy allows, how much the copay is per session, whether or not you have a deductible and how much of that has been met, and whether or not mental health services require preauthorization. If preauthorization is required, you will need to call your insurance provider and request authorization for therapy. They will give you an authorization number and let you know how many sessions they are authorizing. Bring all of this information with you to your first session. We will bill your company directly for services, but we require payment of the copay at each session. If you have not met your deductible for the year, then you will be responsible for full payment of each session until the deductible is met.
